Eligibility Criteria for ADWC Program

i. Resident of Punjab province.
ii. Possess a valid CNIC or B-Form, or valid CNIC of guardian in case of minor or intellectually impaired (A copy of the identification document must be attached with the application).
iii. Disability Assessment Certificate issued by the Disability Assessment Board, in case of wheelchairs, prosthetics, orthotics, and other assistive devices.
iv. A needs assessment detailing the specific needs and recommendations:
a) For hearing aids, recommendations from ENT Consultant/Specialist of DHQ Hospital/Teaching Hospital along with the application.

Wheel Chair

Types of Wheelchairs Provided

Eligibility Criteria for Free Standard Wheelchairs

1. Age Requirements

✅ Minimum age: 3 years.

2. Medical and Disability Criteria

✅ Must possess a valid Disability Certificate.
✅ Body weight must be less than 150 kg.
✅ Paraplegic individuals with lower limb muscle power of less than 2/5.
✅ Must have upper limbs (for self-propelling the wheelchair, if required).
✅ Upper limb function must be intact, with a power of more than 3/5.
✅ Individuals with joint contractures (stiffness in the joints restricting movement).
✅ Patients suffering from severe osteoarthrosis (with muscle power less than 2/5).
✅ Individuals with chronic diseases affecting mobility.

Eligibility Criteria for Electric/Motorized Wheelchairs

Individuals applying for a motorized wheelchair must have paralysis or significant weakness in all four limbs. This means that they cannot effectively move their arms or legs, making it impossible for them to operate a manual wheelchair. Additionally, the power in the upper limbs should not be less than 2/5 or greater than 3/5. This ensures that the applicant can control the electric wheelchair using minimal hand movements if necessary but does not have enough strength to use a manual wheelchair independently.

Reservation of Funds

80% of the fund are allocated for general population and 20% for senior citizens.

Hearing Aid- Eligibility and Fund Allocation

The Punjab Government, under the Assistive Devices Welfare Program, has introduced free hearing aids for individuals with moderate to severe hearing impairment (50-80 dB). Age-Based Fund Distribution Ratio for Hearing Aid

Elderly individuals above 60 years: 10% of the funds are reserved for senior citizens, allowing them to maintain better social engagement and quality of life.

Children aged 1-5 years: 35% of the total funds are allocated to infants and toddlers, as early hearing intervention is critical for speech and language development.

Children aged 5-16 years: 25% of the funds go to school-aged children to support their education and social interactions.

Prosthetics

Fund Allocation for Prosthesis

The Punjab Government, through its Assistive Devices Welfare Program, has allocated 15% of the total program funds for the provision of prosthetic limbs to individuals with physical disabilities. Breakdown of Fund Allocation for Prosthetics

The 15% budget for prosthetics is divided into the following categories:

1. Conventional Above-Knee Prosthesis – 35%

🔹 35% of the allocated funds go towards providing above-knee prosthetic limbs for individuals who have lost a leg above the knee due to injury, accident, or medical conditions.

Tricycle

Tri-Motor Cycle

Who is Eligible for Other Assistive Devices?

To qualify for assistive devices, an individual must meet the following conditions:

✅ Must be in Good Condition (Physically Stable)

• The applicant must be in a stable physical condition to use the assistive device effectively.

✅ Good Life Expectancy (More Than 3 Years)

• The applicant must have a life expectancy of more than three years to ensure that they can use and benefit from the assistive device for an extended period.

✅ Must Have Respective Fitness

• The individual must have the physical ability to use the specific assistive device they are applying for.
